The "Team Banner Information Form" is a way of 
getting the info we need for your banner,
colors, design #, names of coaches players etc. 
It does NOT constitute an order. You must
continue to Step 3 "Pay for Your Team Banner"

Step 3. Pay for your Team Banner

    Check Spellings of ALL Names.
They will appear on banner EXACTLY as written.

Hallmark Airbrush will not be held responsible for misspellings, typing errors or omissions
on the part of the person submitting the order. ALTHOUGH, I have been doing this for some
time and have seen every name spelling imaginable, if a name appears to be grossly misspelled
or beyond explanation, and I can't contact you, I will leave the name off the banner. 
I can ADD NAMES, I CANNOT REMOVE NAMES........and you'll thank me for this advice!

 Please use the same name here and at check out when you pay for your banner.
